7 Add a form
This lesson covers how to collect valuable information from your audience by adding a lead generation form to your quiz. We will:
- add a form block to capture names and emails
- write a text that invites users to submit
- include a GDPR-compliant checkbox with a link to terms
- prepare the form to support segmentation and follow-ups

Hi there, welcome back.
We’ve focused so far on what information and advice you share with your audience via your personalities, via your questions. But now let’s look at the other side — the information that your audience can share with you.
So that you get to know who your audience is, and in this case what your customer cares about and what they like, we’ll do this via a form. That way, you’ll be able to connect the answers the participants give to a name and email address, and you can then use these to collect leads in the first place, segment them, and follow up with personalised or customised emails and offers.
Okay, so this is what our form looks like. Let’s go into the creator and set this up.
Under your blocks, click on Add a block and then Make a form.
By default, a form block includes name and email fields already — perfect. You could also add them separately, but this two-in-one block works well for now.
Now, let’s set the expectation. I want to let participants know that their best-fitting car result will arrive in their inbox. So I’ll write a message like: “Fill out the form to receive your ideal car.”
If we preview this, that’s what the audience will see.
There’s one more thing — the checkbox. This is really important for companies that need GDPR or data compliance. Let’s add a checkbox where participants can agree to your terms.
Scroll down, click on Add form block, and select Checkbox. Then click Edit and write something like: “I agree to the terms and conditions.” You can hyperlink “terms and conditions” to your own privacy page or legal page. Leave the “open in new tab” option enabled. Then click Save.
Now we’ve got a complete form setup.
So in the creator, we’ve now got everything set up — personalities, a title page, questions, and now our form, which means that you can collect this data, email and name, from your audience so that you can follow up with different offers or different cars, for example, in this case. Or you can just build up your list of leads.
